What Is Glutathione and How Does It Work
Glutathione (GSH) is a tripeptide built from three amino acids - glutamate, cysteine, and glycine - that your body synthesizes on its own in nearly every cell, with the liver as the primary production site. It is not a foreign compound or a novel research chemical; it is one of the most abundant molecules in human cells, and levels naturally decline with age, chronic illness, poor diet, and oxidative stress.
Mechanistically, glutathione cycles between a reduced form (GSH) and an oxidized form (GSSG) as it neutralizes reactive oxygen species like superoxide anions, hydrogen peroxide, and singlet oxygen. It also regenerates other antioxidants - including vitamins C and E - after they've been spent, and it serves as the required substrate for glutathione-S-transferase enzymes, the Phase II liver pathway that conjugates and clears heavy metals and other toxins. This dual role, direct radical scavenging plus a central position in the liver's detox machinery, is why it's often described as the body's "master antioxidant."
Glutathione is sold and studied in several distinct forms, and they are not interchangeable: standard oral capsules/tablets (substantially broken down by digestive peptidases before absorption), liposomal or sublingual oral formulations (designed to improve gut survival, though standardized research dosing is limited), IV/injectable glutathione (administered directly into the bloodstream, bypassing digestion entirely), and topical formulations marketed for skin brightening (glutathione inhibits the enzyme tyrosinase, which is the basis for cosmetic skin-lightening use, though this is a distinct and separately evaluated indication from antioxidant or detox claims).

Context: Oral bioavailability of glutathione is genuinely debated in the literature. As a peptide, standard oral GSH is substantially degraded by gut peptidases before it reaches circulation, and controlled trials have shown mixed results on whether oral supplementation meaningfully raises systemic glutathione (see What Research Shows below). IV administration bypasses this absorption question but introduces its own regulatory and sterility considerations.
Regulatory Status
Glutathione's regulatory position is genuinely different from most of the research peptides covered elsewhere on this site, and that distinction matters. Oral glutathione is a legal, over-the-counter dietary supplement in the United States under the Dietary Supplement Health and Education Act (DSHEA) - it does not require a prescription and is widely sold in retail and supplement channels, though like all dietary supplements it is not FDA-evaluated to diagnose, treat, cure, or prevent any disease.
IV and injectable glutathione sit in a different position: they are not FDA-approved for any indication, including detoxification, immune support, or skin lightening. In February 2019, the FDA warned compounding facilities against using certain L-glutathione powder in sterile injectable preparations after a cluster of patients experienced adverse events - including nausea and difficulty breathing - linked to excessive bacterial endotoxin levels in the raw material. That warning underscores a real quality-control risk specific to compounded sterile injectables, separate from any question about glutathione's biological role. Consult a qualified healthcare provider before pursuing any IV glutathione protocol, and verify the sourcing and sterility practices of any clinic offering it.
What Research Shows
Human evidence on oral glutathione supplementation is mixed and appears to depend on dose, duration, and the population studied. A randomized, double-blind, placebo-controlled trial in 40 healthy adults tested 500mg of oral glutathione twice daily for four weeks and found no significant differences between groups in blood glutathione status or in urinary biomarkers of oxidative stress (F2-isoprostanes, 8-OHdG) - suggesting that at this dose and duration, oral supplementation may not meaningfully move the needle in healthy individuals.
A separate, longer randomized trial told a different story in a different population: over six months, 500mg/day of oral glutathione in older adults with type 2 diabetes was associated with significant increases in blood glutathione, reductions in the oxidative-DNA-damage marker 8-OHdG, and improvements in HbA1c, with the largest effects in patients over 55. That combination - a negative result in healthy adults over four weeks, a positive result in an older, metabolically compromised population over six months - is a useful illustration of why single-study headlines about glutathione supplementation should be read cautiously.
